55-year old, Vimala Devi conducted a vermillion-based celebration at her home by inviting married women from her neighbourhood in JuiNagar biannually. In early 2018, she took help from her daughter to conduct this programme. Sitadevi, one of her regular invitees, became close friend of Vimala with whom she had strong bonding. She noticed that Vimala had difficulty in doing even routine household chores.
At the end of the programme when everyone left, Sitadevi stayed back. She informed Vimla that compared to last six months, her droopy eyelids had become even more prominent and that she needs to visit an eye doctor. Vimala hesitantly nodded and admitted that her eye problem has progressed to the extent of inability to open her eyes properly and she has to lift her head to see properly.

Dr. Akshay Nair, an Oculoplastic and Eye Cancer Surgeon at AEHI examined Vimala’s eyes. He found that the droopiness of her eyelids (ptosis) was severe. He diagnosed her as a case of severe aponeurotic ptosis.
In Vimala devi’s case, the droopiness of the eyelid was severe and hence restricted her visual field drastically. She needed to raise her head to be able to see.
Aponeurotic ptosis is the commonest type of ptosis. It is also called senile ptosis. It usually occurs in the elderly. Secondary age-related changes in the aponeurosis of the levator muscle leads to this.
Congenital aponeurotic ptosis is uncommon, but this could be secondary to trauma by forceps delivery, vacuum extraction, etc There are multiple factors that can cause disinsertion of the levator aponeurosis, such as continuous rubbing of the eye, chronic use of contact lenses, inflammatory diseases, trauma or following eyelid or intraocular surgery. Approximately 6% of patients following cataract surgery develops aponeurotic ptosis.
Dr. Nair advised Vimala to undergo Ptosis surgery. Further, the pros and cons of the surgery were also thoroughly explained.
After Vimala’s go ahead, ptosis surgery was performed to lift her eyelids.
A week post-surgery, Vimala and her daughter thanked Dr. Nair wholeheartedly for bringing back the symmetry in Vimala’s eyelids. She no longer needed to lift her head to see the world.
