Avinash Bedekar* was a happy child growing up with his loving joint family. His grandmother used to feed him as soon as he reached home from school and he would tell every detail about his day to her. However, his family was ever worried about his poor vision, constant movement of his eyes and his light sensitivity. They have tried their best to raise him like a normal kid. But as Avinash was growing up, he was becoming more and more aware of his poor vision and his inability to see the blackboard from far. So, the family decided that it is time to take a proper opinion from a good eye doctor.

Avinash underwent detailed eye check-up at AEHI. Instant visible observation was Nystagmus that is related to the eye motility. Retina examination showed that he had Albinotic Fundus.

Since retina is supported with blood vessels and oxygen by choroid, patients with Albinotic Fundus have poorly pigmented retinal layer and clear choroidal vessels.

Nystagmus is an eye condition wherein eyes make involuntary and repetitive movements – side to side, up and down or in circular motion. Such movements hamper vision by diminishing it and affecting the balance and coordination. The most common cause of nystagmus in children is infantile nystagmus syndrome (INS). Nystagmus may be associated with foveal hypoplasia, albinism, congenital cataracts, corneal opacities, retinopathy of prematurity, etc.

Dr. Prachi Agashe, a Paediatric Ophthalmologist and a Squint Surgeon at AEHI observed that Avinash’s fovea was underdeveloped. She then diagnosed him with Oculocutaneous albinism.

She recommended him new prescription for spectacles with tinted glasses along with prism and asked him to follow up after two months.

Oculocutaneous albinism is referred to a set of rare genetic disorders that may pass onto the next generation wherein the skin, hair and our eyes are completely devoid of melanin pigment. This condition occurs due to mutation in that particular gene which produces this pigment in the cells – melanocytes.

Avinash came for follow up after six months. Although, there was small chin depression, there was no face turn. Follow up session for Avinash was then scheduled after next two months and he was advised to continue using the same glasses.

On the follow up day, it was observed that Avinash’s abnormal head posture wasn’t improving, Dr. Agashe then recommended him to undergo nystagmus surgery.

Nystagmus surgery is performed to correct abnormal head posture. This procedure is aimed at decreasing nystagmus amplitude or frequency by involving extraocular muscle recession, resection, etc or combination of these.

Avinash underwent Nystagmus surgery and followed up after a month. After a month of surgery, he showed two-line improvement in the (Snellen chart) visual acuity and with no abnormal head posture.

With the subsequent follow up, Avinash also showed good eye motility. It is very important that any child who has abnormal eye movement undergoes an eye evaluation with the paediatric eye doctor. Right treatment at the right age can go a long way in ensuring the best possible visual outcome for these kids.
