50-years-old Vinod Mehta* was a spice exporter who traveled all over Maharashtra with his team. Once while driving from Navi Mumbai to Nashik, they took a short break and came across a hoarding emphasizing the importance of regular eye check-ups by Advanced Eye Hospital and Institute (AEHI), Sanpada, Navi Mumbai.
This prompted Vinod to take an appointment at AEHI, Sanpada. When Vinod’s eyes were examined, his sharpness of vision (visual acuity) was found to be normal. However, when his eye pressures were tested they revealed raised eye pressure in both eyes (28 and 30mmHg as opposed to the normal 8-21mmHg).
Glaucoma is a set of eye disease where the optic nerve is damaged typically due to raised eye pressures. Considering this fact, Vinod was referred to Dr. Rajesh Mishra, an eye doctor specializing in Glaucoma surgery and treatment of AEHI.
Vinod was further evaluated by Dr. Rajesh Mishra.
Gonioscopy test was done for him, which studies the area from where fluid is drained out of the eye (called the angle). This test is usually done for all patients who are suspected for Glaucoma.
Glaucoma can be of two types depending on whether there is a block in this area (called closed angle / open angle glaucoma).
Unexpectedly, Vinod’s angle too turned out to be normal. Subsequently, optic nerve examination was done. This test was done to check the vertical cup to disc ratio (this ratio reveals the damage to the optic nerve as seen in glaucoma) which was 0.5 and anatomically normal. . Also, there seemed to be no defect in the nerve fibre layer of the retina too.
Since, Vinod showed high eye pressure, the thickness of his cornea was checked. This is performed because, sometimes, thin corneas may give a false reading or lower value of intraocular pressure and vice versa. Tests showed that his corneal thickness was within normal range. .
Then, his visual fields were then tested to see if there was any decrease in the peripheral (side) vision. This turned out normal even with a repeat test two days later.
Vinod was diagnosed as Ocular Hypertension.
Ocular Hypertension is when the pressure in one’s eyes is higher than normal (more than 24 mm hg in both the eyes). Ocular hypertension is not the same as glaucoma. Some people can have normal optic nerve despite having high eye pressure. However, if ignored, this can lead to glaucoma or even permanent vision loss in some others. Hence, people with ocular hypertension are considered as ‘glaucoma suspects’.
Following are the risk factors which may be associated with Ocular Hypertension and need to be reviewed:
- Older age
- Eye trauma
- Higher IOP
- Larger cup:disc ratio
- Thinner central corneal thickness
- Pseudoexfoliation syndrome
- Medications containing Steroids
Dr. Rajesh then prescribed him few eye drops and asked him to review after a few months.
It is important to know that ocular hypertension can happen to anybody above 35 years of age. And since it does not give any outward symptom, it is important to undergo eye check-up to diagnose it early and avail treatment before Glaucoma hits us.
