18-year-old Karan Thappar* had droopy eyelids because of which many of his friends made fun of him. He was determined to not be a laughingstock amongst his new friends at university. Ice pack test for eyelid was conducted for Karan. Ice pack test is a safe, inexpensive, and quick to perform procedure. In this test, a covered pack of ice is applied to the eyes for 2–5 minutes. If the patient shows improvement in the eyelids level by more than 2mm after the ice pack is removed, the result is deemed to be positive.
For Karan, the ice pack test for eyelid was negative, meaning he did have congenital ptosis in his eyes.
Moreover, the function of his eye muscle – Levator palpebrae superioris (LPS) which is responsible for eyelid elevation was also good.
Further, he had no double vision (diplopia). His extraocular motility was full. His upward and outward eye movement was also good- a condition known as Bell’s phenomenon.
In Karan’s case, the droopiness of the eyelid also affected his visual field, thus giving him restricted vision. So, Dr. Nair advised him to undergo Levator Resection Surgery which will cosmetically correct eyelid level as well as the field of vision. Further, the pros and cons of the surgery were also thoroughly explained.
After Karan’s father was convinced, ptosis surgery was performed subsequently.
Levator Resection Surgery is a surgery wherein an incision (cut) is made through eyelid crease to locate the levator muscles and then rupture it from the connective tissue of the eyelid i.e. tarsus. Then, levator is shortened and reattached to the tarsus. After about a fortnight, the external stitches were removed.
Karan had symmetrical eyelids and this renewed his self-esteem and confidence.
