61-year-old Swati Agarwal, an Airoli native, was a happy librarian who worked diligently everyday as she was passionate about books.
But soon her work was becoming a burden as she used to experience grittiness, burning sensation and fluctuating vision in her eyes especially when she used to read and use computer. But, as a natural human tendency, she assumed these signs are due to fatigue. Finally when her symptoms worsened, her daughter insisted her to visit an eye doctor in Navi Mumbai for an eye check-up. She started searching on the internet and after reading numerous positive feedbacks and reviews from happy patients, she chose Advanced Eye Hospital & Institute (AEHI) in Sanpada.
Initial comprehensive eye check-up was done for her. The visual acuity test confirmed that although she had normal vision but it was fluctuating. She mentioned about the symptoms of irritable and gritty eyes. Dry eye evaluation was one for her. The Schirmer’s test score was 2 mm and 3 mm after 5 min for right and left eye respectively.
Schirmer’s test is a dry eye test where the moisture content in the eyes or production of tears is determined using paper strips. Paper strip is inserted under the lower eye lid into the eyes. When the test result is normal- the filter paper shows more than 10 mm of moisture in 5 minutes. For Swati the reading was too less and was indicative of dry eyes.
Swati was then referred to Dr. Vinayak Damgude, a Cornea and Cataract surgery eye doctor at AEHI. On probing about her ocular history, she informed that her family members have observed that her eyes are half closed whenever she is in deep sleep (an eye condition called – lagophthalmos). Additionally, Dr. Vinayak used a special stain to check the severity of her dry eyes. He noticed that the cornea in both her eyes showed extensive staining suggestive of severe dry eyes and that explained her symptoms as well. Dr. Damgude diagnosed her with dry eye syndrome and mild lagophthalmos. He advised her some blood tests to rule out any auto-immune cause for the dry eyes.
Aqueous tear deficiency happens when its source – lacrimal gland secrets tears less than the optimal level. This low-level of tear secretion is mainly due to abnormal functioning of the lacrimal glands which can happen due to multiple reasons. Reduced tear production leads to dryness in the eyes along with irritability and grittiness.
Lagophthalmos is a type of Exposure-related Dry eye syndrome. This happens when your eyelids are not positioned properly or is not functioning optimally or it has some structural defect. Luckily in case of Swati it was mild and didn’t have much effect on her eyes.
Dr. Vinayak prescribed her a few tear substitute eye drops, eye ointments and anti-inflammatory eye drops. Swati followed for next couple of weeks. He also advised her to restrict computer and mobile activity to the bare minimum.
Gradually, Swati experienced huge relief from gritty eyes and she could read on the computer without any eye problem. However as a precaution she reduced the gadget use to prevent these symptoms form recurring.
It is not uncommon that hot season along with excessive computer and mobile use can lead to dry eye. Dry eye in turns causes gritty eyes, burning sensation, conjunctivitis and eye tiredness. There are many times when we self-treat to get relief from dry eyes. But, like Swati, it is important to understand that an eye check –up is important to ascertain the type and severity of dry eyes and treat accordingly.
