Peripheral Iridotomy for Occludable Angle Glaucoma Treated at AEHI, Navi Mumbai

Mr Kohli (Name Changed) 65 Years male a retired professor resident of Navi Mumbai visited Advanced Eye Hospital and Institute (AEHI) located at Sanpada, Navi Mumbai  with Complaints of Redness, Watering, swelling in his left eye since 3 days. Being a professor he has spent most of his time in reading books and teaching various classes. Due to his eye complaints he found difficult to read books and also teach a class. He couldn’t focus on the board well and his eyes would start watering which made it difficult to carry out a lecture.


Mr Kohli was worried and he decided to ask few colleagues and friends about the best eye hospital for the eye treatment .Mr Kohli’s friend has undergone Cataract operation at AEHI. As he entered Advanced Eye Hospital and Institute he was welcomed nicely by the staffs at AEHI, which created a pleasant environment for him. Then he got his routine eye evaluation, the optometrist referred him to Dr. Rajesh Mishra, Glaucoma and Cataract Surgeon at AEHI. Dr. Rajesh Mishra checked his eyes and noticed that the pressure in his left eye is 44 mm Hg, and on examination he noticed oedema (swelling) in his cornea along with congestion. Further Dr. Rajesh Mishra advised and  carried Gonioscopy test to measure the angle between the cornea and the iris. Dr. Rajesh Mishra diagnosed the condition as Left Eye Pseudo exfoliation Glaucoma with Early Cataract in his both eyes. But his Left eye was more affected than his Right Eye.

Gonioscopy: It is a test for Glaucoma which measures the angle between the cornea (the outer transparent dome of the eye) and the iris (the coloured part of the eye). This angle is the drainage area from where the fluid drains out of the eye. Seeing if this angle is open or close gives an idea about the type and severity of glaucoma.

Dr. Rajesh Mishra explained them in detail about Pseudo exfoliation Glaucoma.  Dr. Rajesh Mishra said Mr Kohli that a laser operation (Peripheral Iridotomy) is required for this kind of eye disease.

What is Pseudo exfoliation Syndrome?

It is a systemic disease with primarily ocular manifestations characterized by deposition of whitish gray protein on the lens, iris, ciliary epithelium, corneal endothelium and trabecular meshwork.

Dr. Rajesh Mishra  asked Mr Kohli to focus in a beam light so that he can create a hole on the outer edge  of the iris or the coloured part of the eye, this hole allows the fluid to flow between the anterior chamber , the posterior chamber , front part  and the area behind the iris of the eye. This helps in decreasing the pressure in the eyes. Thus Dr. Rajesh Mishra operated on his left eye (Peripheral Iridotomy) and administered few eye drops for his Glaucoma.

Peripheral Iridotomy:- It is a procedure that is performed in the eye to treat angle closure glaucoma. 

Mr Kohli came for his follow up after 5 days, his eyes were examined in which the pressure in his left eye was 10 mm Hg (reduced), and the redness, congestion and swelling too were decreased. Mr. Kohli was very happy after the laser peripheral iridotomy. He was feeling absolutely normal and eye became painless, and free from complains. There was improvement in vision clarity too after the laser.
